Please review all application deadlines before submitting them. Late application materials will not be accepted and incomplete applications will not be considered.
Entrance Exam for all ADN Nursing programs
The HESI Nursing Entrance Exam is an admission requirement for all associate degree Nursing programs. The entrance exam provides evidence of an applicant’s academic skills in:
• Reading (ADN applicants must score 80 or above to meet admissions criteria.)
• Grammar (ADN applicants must score 80 or above to meet admissions criteria.)
• Vocabulary (ADN applicants must score 80 or above to meet admissions criteria.)
• Math (ADN applicants must score 80 or above to meet admissions criteria.)
• Anatomy & Physiology (ADN applicants must score above a 80 to meet admissions criteria.)
Entrance Exam for our LPN program
The HESI LPN Entrance Exam is an admission requirement for the LPN certificate program. The entrance exam provides evidence of an applicant’s academic skills in:
• Reading (LPN applicants must score above a 80 to meet admissions criteria.)
• Grammar (LPN applicants must score above a 80 to meet admissions criteria.)
• Vocabulary (LPN applicants must score above a 80 to meet admissions criteria.)
• Math (LPN applicants must score above a 80 to meet admissions criteria.)

- Applicants must submit documentation of the HESI Entrance Exam results for consideration. Alternative exams, such as the TEAS, may not be substituted for the HESI exam.
- Applicants may take the exam at MCHP or another testing site. If not taken at MCHP, applicants must have an official copy of the test results sent to MCHP from the testing site or by ordering the results from HESI directly.
- Applicants may retake the HESI exam, for an additional fee, one time per admission cycle. Applicants may retake the entire exam or only the section(s) of the exam they decide to re-test. HESI test scores are valid for one year from the test date.
- Applicants may make an appointment to take the exam at MCHP by completing the registration form and sending it with payment to the address below. There is a $50 non-refundable fee for the test. Space for testing is limited and registration will be processed on a space-available basis. Registration confirmation will be sent by email.

Radiologic Technology Entrance Exam
The Accuplacer exam is an admission requirement for Radiologic Technology. The entrance exam provides evidence of an applicant’s academic skills in:
• Reading
• Writing
• Quantitative Reasoning, Algebra and Statistics
Health Sciences Entrance Exam
The Accuplacer exam is an admission requirement for Health Science applicants. The entrance exam provides evidence of an applicant’s academic skills in:
• Reading
• Writing
• Math and Algebra
- Accuplacer scores are valid for one year from test date.
- Applicants may take the exam at MCHP or another testing site. If not taken at MCHP, applicants must have an official copy of the test results sent to MCHP from the testing site.
- Applicants may retake the Accuplacer exam, for an additional fee, one time per admission cycle. Applicants may retake the entire exam or only the section(s) of the exam they decide to re-test. Test scores are valid for one year from test date
