Course Information/Admission Requirements
Applications will be accepted until January 5, 2018 or until the course fills, whichever comes first.
Completed Application, including $50.00 application fee.
Applicants must be high school graduates or have their HiSET or GED or receive a grade of “50” or higher on the Reading Section of the Accuplacer Assessment Exam. Applicants must have an official copy of their high school transcript sent directly from the high school to MCHP. Official results, sent from the high school/adult education program or state, from a HiSET or GED exam must be submitted if you are not a high school graduate or have not received a diploma from a State approved secondary educational institution. If you have not graduated from high school nor have your HiSET or GED, please contact Erica Watson for information on the Accuplacer assessment exam. (207) 795-2843.
Must be at least 18 years of age.
If accepted you will be required to provide the following:
4) Submit Background Check Authorization, including $65.00 background check fee.
5) Provide Student Immunization Record and complete and sign the Student Physical Requirement form. The complete list of immunization requirements will be sent if you are accepted into the program. This form shall be returned to Joan Anderson, RN either per email: firstname.lastname@example.org, fax: 795-2849, or regular mail.
Deadline to submit this completed form: February 8th
6) CPR Certification (submit copy of front and back of document by February 8, 2018.)
CPR certification must be one of the following:
American Heart Association Basic Life Support Provider course OR
American Red Cross Basic Life Support for Healthcare Providers course
